What is a Community Paramedic?
A community paramedic is a highly trained EMS professional who goes beyond traditional
emergency response. Instead of only responding to emergencies, community paramedics
provide proactive, non-urgent healthcare services directly in patients’ homes. This may
include monitoring chronic conditions, providing post-hospitalization follow-up care,
conducting wellness checks, and connecting patients with community resources. The goal
is to improve patient outcomes, reduce unnecessary hospital visits, and support patients
in managing their health independently.
